The Leonardo wants your child’s summer camp experience to be safe, fun and healthy. However, in the event of an accident or illness, it is important that we have the following information. In order to finalize your registration, complete each page of this form.

This form includes the following:

• Liability Waiver and Release
• Consent For Use Of Name, Photograph and Statement (optional)
• Camper Pick-up and Drop-off Authorization
• Camper Self-Release Authorization – Ages 13 and up only (optional)
• Emergency Contact and Medical Information

The Leonardo reserves the right to cancel your registration without refund if these forms are not submitted within two weeks of registration. Please complete it now.
